事故や災害が起きると、私たちはよく責任者を探して批判をぶつけます。それはただのうっぷん晴らしで、有益ではありません。感情が先走って理論が置き去りになっています。誰でも非難されるのは避けたいものです。非難されると、非難を避けようとして事実を隠すことになります。ということは、責任者を非難することは、真相解明の邪魔になりかねません。
誰でも、大切な見落とす可能性があります。したがって、責任者を非難したり個人の注意力に頼るよりも、被害を最小限に抑えるシステムを構築することの方が大切です。

At an accident or disaster, we often search for the responsible to accuse them. It is useless, just letting out our anger. If we do it, we put our emotions behind logic. Everyone wants to avoid being blamed. When we receive accusations, we hide some facts to try to avoid blame. Thus, accusing the responsible means disturbing to disclosure of truth.
Anyone can overlook something important. Therefore, it is more important to build systems to minimize damage than to blame the responsible and rely on each individual's attention.
